INTERDISCIPLINARY APPROACH TO THE MANAGEMENT OF A PATIENT WITH ACUTE ENTEROMESENTERIC THROMBOSIS: CASE REPORT

Introduction. Acute enteromesenteric thrombosis is an abdominal vascular drama with severe consequences and a reserved prognosis.
Case presentation. In this paper, we report the case of a 73-year-old patient, urgently admitted to the hepatobiliopancreatic surgery department of the Republican Clinical Hospital "Timofei Moșneaga", Chișinău, Republic of Moldova. Following the clinical and paraclinical assessment (interdisciplinary consultation, transabdominal ultrasound, abdominal computed tomography in angiographic mode), the diagnosis of acute enteromesenteric thrombosis is established. The patient is proposed for surgical treatment, the urgent intervention in common agreement with the patient being: laparotomy, thrombus extraction from the superior mesenteric artery with the restoration of intestinal viability. The particularity of the case is represented by the multidisciplinary approach, early diagnosis and urgent surgical intervention, essential elements that facilitated the favorable postoperative evolution.
Conclusions. Acute enteromesenteric thrombosis is a rare but severe abdominal vascular emergency that requires early recognition and rapid therapeutic intervention. In the presented case, prompt imaging diagnosis and efficient collaboration between specialties allowed the application of surgical therapy with a margin of safety and favorable outcome. This case highlights the need for early evaluation with a multidisciplinary approach and the use of CT angiography as a safe and effective method in the early diagnosis of TEM.
