PANCREATIC NECROSIS IN TERMINAL DISEASES WITHOUT SURGICAL IMPACT

Abstract

Objectives. Pancreatic necrosis are encountered incidentally during the anatomopathological examination in terminal non-surgical conditions. Elucidation of their frequency, morphological aspects, and background pathologies constituted the purpose of the given study.

Methods. From anatomopathological investigation protocols, collected from Municipal Clinical Hospital ”Saint Arhanghel Mihail”, during 2014 – 2020, of 550 cases of death, caused by diseases, in case of patients who did not undergo surgery, were selected 198 (36%) cases with which included changes at the pancreatic level, confirmed morphologically. Subsequently, the clinical observation sheets of these patients were also examined.

Results. The obtained morphological results highlighted: a) extensive pancreatic necrosis; b) focal pancreatic necrosis on the background of healthy pancreas; c) focal pancreatic necrosis on the background of chronic pancreatitis; d) pancreatic autolysis. In the cases of terminal diseases, liver cirrhosis predominated, followed by degenerative cardio-pulmonary diseases, cancer, inflammatory diseases mainly of the lungs, and diabetes.

Conclusions. In serious, terminal cases, in which death occurs after a long period, it is attested, both diffuse and localized pancreatic necrosis occur with obscure symptoms.
